Who is this transportation service for?
Residents of Centre, North or East
Wellington who do not have access
to transportation and meet eligibility
criteria for service.
Target populations for this service
include: seniors, adults with a disability,
low income families and individuals.
Is there a cost for this service?
Individual service providers may charge
fees based on client assessment criteria.
Sliding scales are utilized to determine
individual client fees based on their
financial situation.
How do I access this service?
Call 1-866-694-9562 or 519-843-7003.
Contact our Transportation Co-ordinator
Christine Booker, or Ali Kane , answering
a few short questions.
Your needs will be assessed and you will
be referred to a direct provider in your
community.
Connection to that provider will be made
where the assessment process will be
completed.
Once you are receiving service all future
requests are made directly to that
provider. If your needs change or service
provider is no longer able to meet your
needs you will be referred back to central
referral for an additional match.

What types of destinations qualify?
Necessary appointments: medical, social
services, legal.
Social and Healthy living: visiting family or
friends in long term care, participation in
social or active lifestyle programs.
Daily living: grocery shopping, banking.
Is there an assessment process?
Assessments are done by service
providers at the time of intake to
understand the needs of the client, ensure
appropriate service is provided, address
any additional concerns the client may
have.
These are completed in the client’s home
or over the phone depending on the needs
of the client and the type of information
required by the service provider

on volunteer availability however our best
efforts are made to fulfill all requests.
